PROPOSAL FOR THE GLOBAL FUND FOR UNIVERSAL HEALTHCARE ACCESS ACT

Preamble


WHEREAS, access to quality healthcare is a fundamental human right, essential for human dignity, well-being, and socio-economic development;
WHEREAS, vast disparities in healthcare access and outcomes persist globally, exacerbated by poverty, conflict, climate change, and weak health systems, leaving billions without essential medical services;
WHEREAS, global health crises underscore the interconnectedness of our world and the imperative for collective action and solidarity to protect all populations;
WHEREAS, a strong welfare state and robust social safety nets are cornerstones of social democracy, ensuring that no individual is left behind due to illness or lack of financial means;

NOW, THEREFORE, the World Parliament, committed to fostering a more just, equitable, and healthy world, hereby establishes the Global Fund for Universal Healthcare Access.

Article 1: Establishment of the Global Fund for Universal Healthcare Access (GFUHA)


1. A permanent international financial mechanism, to be known as the Global Fund for Universal Healthcare Access (GFUHA), is hereby established.
2. The GFUHA shall operate under the auspices of the World Parliament, in close collaboration with the World Health Organization (WHO) and other relevant international bodies.

Article 2: Objectives of the GFUHA


The primary objectives of the GFUHA shall be to:
1. Achieve Universal Health Coverage (UHC): Support member states, particularly low and middle-income countries, in developing and strengthening their primary healthcare systems to ensure all individuals have access to the health services they need, when and where they need them, without financial hardship.
2. Strengthen Health Infrastructure: Invest in the construction, renovation, and equipping of healthcare facilities, with a particular focus on rural and underserved areas.
3. Develop Human Resources for Health: Fund training, recruitment, and retention programs for healthcare professionals, including doctors, nurses, community health workers, and specialists.
4. Ensure Access to Essential Medicines and Technologies: Support procurement, distribution, and local production of affordable, quality-assured essential medicines, vaccines, diagnostics, and medical technologies.
5. Reduce Out-of-Pocket Expenditure: Implement mechanisms to reduce catastrophic health expenditures and impoverishment caused by healthcare costs, moving towards pre-payment and risk-pooling systems.
6. Promote Health Equity: Prioritize interventions that address health disparities, focusing on vulnerable populations, including women, children, the elderly, persons with disabilities, and marginalized communities.
7. Support Research and Innovation: Invest in global health research and development, especially for diseases disproportionately affecting low-income countries, and promote the equitable distribution of innovations.

Article 3: Funding Mechanisms


1. The GFUHA shall be financed through a progressive and diversified funding model, recognizing shared global responsibility:
* Mandatory Member State Contributions: A portion of each member state's Gross Domestic Product (GDP) shall be contributed annually, with a progressive scale ensuring higher-income nations contribute a larger percentage.
* Global Progressive Taxation: Explore and implement innovative global taxation mechanisms, such as a modest Financial Transaction Tax (FTT) or a Carbon Tax, with a dedicated portion allocated to the GFUHA.
* Voluntary Contributions and Philanthropy: Encourage and accept additional voluntary contributions from member states, private foundations, corporations, and individuals.
* Debt Relief for Health: Advocate for and facilitate debt relief initiatives for developing countries, with savings redirected to health system strengthening.

Article 4: Governance and Accountability


1. The GFUHA shall be governed by a Governing Board, comprising representatives from:
* Member states, ensuring equitable geographical and economic representation.
* The World Health Organization (WHO).
* Civil society organizations with expertise in health and development.
* Independent health and financial experts.
2. The Governing Board shall be responsible for strategic direction, policy setting, resource mobilization, and oversight of fund allocation.
3. Transparency and accountability shall be paramount:
* All financial transactions and program outcomes shall be publicly available.
* Regular independent audits and impact assessments shall be conducted and published.
* Mechanisms for stakeholder engagement and grievance redressal shall be established.

Article 5: Allocation and Implementation


1. Funds shall be allocated based on a transparent, needs-based, and equity-driven framework, prioritizing countries with the greatest health burdens, weakest health systems, and highest levels of poverty.
2. Recipient countries shall submit detailed national health plans and proposals demonstrating alignment with GFUHA objectives and national health priorities.
3. Implementation shall be country-led, fostering national ownership and capacity building, with technical support from the WHO and other relevant agencies.
4. Emphasis shall be placed on sustainable, long-term investments rather than short-term emergency aid, building resilient health systems capable of responding to future challenges.

Article 6: Monitoring and Evaluation


1. A robust monitoring and evaluation framework shall be established to track progress towards UHC targets and GFUHA objectives.
2. Performance indicators shall include, but not be limited to, improvements in maternal and child health, reduction in communicable and non-communicable diseases, increase in health workforce density, reduction in out-of-pocket health expenditure, and equitable access to essential services.
3. Regular reports on the impact and effectiveness of GFUHA investments shall be submitted to the World Parliament and made publicly accessible.

Article 7: Entry into Force


1. This Act shall enter into force upon its adoption by a majority vote of the World Parliament.
2. All member states are called upon to ratify and implement the provisions of this Act with utmost urgency and commitment.

It embodies the social democratic principles of solidarity, equity, and collective responsibility, ensuring that economic growth serves the well-being of all people.
